The goal of Person and Family Centered Care (PFCC) is to create true partnerships between patients, families, and health care professionals that will lead to better outcomes and enhance the experience, quality, and safety of care. PFCC moves from doing things to and for patients to partnering with patients.

Each of the eight Kaiser Permanente regions has an established PFCC program, collectively engaging with nearly 1,000 members known as Patient Partners. Whether you are looking to engage with a Patient Partner(s) for a one-time event or engage with a Patient Advisory Council, we are here to work with you to make that as efficient and effective as possible. We are committed to quality engagements that lead to the best outcomes whether locally, regionally, or nationally.
